Personal information is any information about an individual that can identify him or her. This can include, but is not limited to a client’s age, social insurance number, health information and financial information.
We need to collect only the information required by our insurance partners so that a life insurance carrier can issue a policy. We will not ask for more information than what is required on the life insurance application. Any information that we do not require will be destroyed in a safe and secure manner.
We need to ask for an individual’s consent before obtaining personal information. The consent applies to the life insurance carrier(s) where the application for insurance is being made. If a client does not provide consent or withdraws that consent, then our representative will not be able to complete the application for insurance. We will only disclose personal information to a third party with consent or when required by law.

Limiting Use, Disclosure and Retention:
We will only use the information as it relates to the application for insurance and only with the insurance carriers in question. Any personal information that is not required will be destroyed. Physical copies will be shredded, or electronic copies will be deleted from our systems. However, the documents required to open your file, process a new application or a new transaction will be retained in electronic format.
We make every effort to ensure that your personal information is accurate and complete.
We have implemented and continue to monitor and implement safeguards so that your personal information remains strictly confidential. We limit access to your information to only those employees that need it. We take physical precautions like locking offices, desks and filing cabinets and electronic measures including firewalls, passwords, and encryption
Request for Access:
You have the right to ask if we have any personal information about you on file and you have a right to access that information. We can provide you with this information within a reasonable period after we have received your written request, usually within thirty days.
We may refuse to provide you with the information you requested if there are references to other individuals or the information cannot be disclosed for legal or other reasons.
